As reported in a previous article, the Annual Report scam being run by a company called Corporate Records Service doesn’t appear to be going away. Once again, in the last few days, added clients have been asking about a form they received in the mail that appears to be the Annual Report form we file for hundreds of our business clients. The mailing from Corporate Records Service requests the business recipient to fill out an “Annual Minutes Record Form” and pay a $125.00 fee. We now have received a copy of what they are sending, so you may click here to see the SCAM Annual Report Form and Instructions for yourself, so you are aware if it is sent to you.
NO SUCH FORM OR FEE IS REQUIRED BY ILLINOIS LAW.
The form looks very similar to the real Annual Report form that IS required to be filed each year with the Illinois Secretary of State. The mailing comes in a green envelope and indicates the $125 fee must be paid by a specific date. Many clients have been fooled by this mailing.
This scam is happening in at least 10 different states based on what we have heard. If you get an “official looking” communication, please contact us. If you have already paid the fee, please contact us so we can get your information to the Illinois Secretary of State’s Business Services Office which is investigating this matter.

Thank you for creating this post… My corporation is formed in the state of New York and I’ve received a 2013 – “Annual Minutes Form” from – “corp-records.com”, which is located in:
Corporate Records Service
911 Central Ave. #260
Albany, NY 12206-1292
When I called and asked about the form, the woman on the phone told me to give her my credit card information right away to start the process. I told her I had time to wait but she kept pushing for it. This got my attention and made me search for them… Can’t believe they had all my information too!
